संजय गांधी पोस्टग्रेजुएट इंस्टीट्यूट लखनऊ (SGPGI) ओपीडी में जाने के लिए आपको पहले ऑनलाइन अपॉइंटमेंट लेना आवश्यक है. करुणा काल को देखते हुए अभी विभाग द्वारा केवल नेगेटिव पीसीआर रिपोर्ट आने वाले मरीजों को ही डॉक्टरों द्वारा देखा जाएगा. मरीज को अपने साथ करो ना नेगेटिव रिपोर्ट लेकर आना आवश्यक है.

मार्च में जारी की गई इस नोटिफिकेशन के अनुसार विभाग 20 नए रोगी एवं 30 पुराने रोगियों को प्रतिदिन देखेगा. इसके साथ ही स्वास्थ्य कर्मियों के लिए भी गाइडलाइन जारी की गई है जिसमें कि उन्हें सामाजिक दूरी बनाए रखना, मास्क का उपयोग करना तथा नियमित हाथ होते रहने जैसे कार्यों का पालन करना अनिवार्य होगा यह पूरी प्रक्रिया 30 मार्च से लागू हो जाएगी.

Patient can do registration / OPD booking for next one month only.
When you you are going to register in your respective department, firstly check the working day of that department.
After registration take a printout of your registration with you.
On your OPD date, arrive before 10:00 a.m. And report on the online registration counter. Show them all your referral documents with your online registration token.

For registration you have to follow the steps which are mentioned below. This complete procedure will be held on the official website of sanjay Gandhi postgraduate Institute of Medical Sciences Lucknow.

To do that you have to visit the official website of SGPGI.
When you land on the official page you will see this is a patient portal.

There are two options are available online registration and OPD schedule.
If you want to check out the OPD timing and doctor details and you need to click on it.
In case you want to do registration then click on online registration.
After click on it you will be redirected to SGPGIMS patient portal. In this portal patient can do online registration and fill the form.
When you reach on this page you will see some instructions which are already mentioned in this post already.
Read all the instructions and then click on the “I have read all the above instructions” and then click on continue button.

Now patient registration form is open on your screen. Firstly choose date on which you want to do online registration.
After that choose department in which you want to visit.
At that time you will see doctor name which is available on that particular date.
The next option is choose your name. First name middle name and last name.
Then enter father name, gender, date of birth and marital status.

The next option you have to choose your religion, occupation, mobile number and email ID.
In the next section you have to provide contact details like your address.
In address enter your house number, street name, locality, country, state, city and pin code number.
Also enter the nearest railway station.

The next correspondence address.
If your details are same then enter these details and that forward to next section that is emergency contact details.
Enter contact person name, phone or mobile number and relationship with contact person.
In case of referral you need to enter referral doctor, referral department and referral Hospital details.
The next section is patient identity information.
In this section we need to choose identity proof like Aadhar card, PAN card or voter ID card. Also provide the identity card number and issue authority name.
After fill all the details, at the end you need to click on the the declaration and then fill the captcha code.

Next click on the register button and then you will be successfully register for SGPGI OPD Appointment.
